Pimozide

Orap

Use with caution

Your body clears pimozide slowly, which could raise the risk of heart rhythm side effects. Half the usual maximum dose is advised.

The genes behind this label

The guidelines read one gene for pimozide. Each card shows the two copies this person inherited and what they mean.

CYP2D6Your two copies: *4/*4

Poor metabolizer

Neither copy works.

0.0 activity scoreTypical: 1.25 to 2.25

Turns on or clears about 1 in 4 prescription drugs. These include many drugs for depression, pain and ADHD.

What the guidelines say

What each guideline says about your result.

CYP2D6Poor metabolizer

In plain words
Your body clears pimozide slowly, which could raise the risk of heart rhythm side effects. Half the usual maximum dose is advised.
What it means
Decreased CYP2D6 activity raises the plasma concentration of pimozide, which theoretically increases the risk of QT prolongation and torsade de pointes. Following the dose limit offsets this.
What the guideline suggests
Use no more than the following doses (50% of the normal maximum dose): 12 years and older, 10 mg/day; younger than 12 years, 0.05 mg/kg per day to a maximum of 2 mg/day.
In plain words
Your body breaks down pimozide more slowly than normal, causing higher levels of the drug in your system. A lower dose is recommended to reduce the risk of side effects.
What it means
Results in higher systemic concentrations. Dosages should not exceed 0.05 mg/kg in children or 4 mg/day in adults who are poor metabolizers and dosages should not be increased earlier than 14 days.
What the label suggests
Do not exceed 0.05 mg/kg a day in children or 4 mg a day in adults, and do not increase the dose earlier than 14 days.
